The GRcade Videogame Industry Hall of Fame: The Survey

In conjunction with The Videogame A-Z Hall of Fame currently running in GGC, I've decided to run a quick survey to find out what should enter The Videogame Industry Hall of Fame (It's in the same building as the A-Z Hall, just down the corridor )

All you have to do is copy/paste or quote the survey below, fill it in completely (Don't send me half-finished forms as that just confuses matters) and PM me your answers. I'll only accept your first set of answers, so don't PM me asking to change your forms after you've sent them. Cheers.
